Underwriters under time pressure write narrative. Reviewers under time pressure need structure. The notes that survive reconstruction name the risk factors considered, the policy clause applied, and whether an override required dual control.
We recommend a lightweight template inside the case system: risk factors observed, mitigating evidence, decision, and override path. Training sticks when the template is shorter than free text—not longer.
If your platform already stores rule-engine scores, surface them next to the human note. Auditors should not have to export three systems to understand one approval.
